Comprehensive Healthcare Support For Medi-Cal Beneficiaries Facing Medical and Psychosocial Challenges
Our goal is to help individuals reduce emergency room visits and hospital readmissions, improve patient outcomes, and promote patient overall physical and mental health.
MEDI-CAL CARE PLANS
We create a care plan for those with Medi-Cal by connecting them to resources that improve their physical health, mental health, substance use, finances, and housing situation. We accompany our clients to doctor’s appointments, educate them about their health, and advocate on their behalf considering their unique cultural and linguistic background.

REQUIREMENTS
- You are 4-17 years old (Children and Youth) or 18+ years old (Adult) and
- You have Medi-Cal under LA Care insurance, Anthem, or Health Net and
- You live in Service Area 4 and
- You meet one of the following criteria:
- You are experiencing mental health challenges or
- You struggle with drug or alcohol use or
- If you are a child or youth with current or history of Child Welfare System involvement
